Call at least 2 business days ahead.
Schedule routine rides up to 30 days before your appointment. Weekends and holidays are not business days—call early.
Get to your medical appointment
Modivcare arranges free non-emergency rides for eligible Trillium Tailored Plan members to Medicaid-covered medical care, including mental health visits. Confirm your plan and trip eligibility when you call. NC Medicaid Direct members arrange transportation through their local DSS.
Have your appointment details ready
Keep your Medicaid ID card nearby. Have your name, pickup address and phone number, appointment date and time, and the provider’s name, address and phone number ready. Explain wheelchair, attendant or other access needs.
Call to request your ride
Dial 877-685-2415 and ask for Trillium medical transportation. Request both the ride there and the ride home.
Confirm and save the details
Ask for the pickup time, trip number and return-ride instructions. Keep the number for ride assistance, changes or cancellation.
Mileage reimbursement may be an option.
If you drive yourself—or a friend or family member takes you to an eligible medical appointment—you may qualify for money back for mileage. Call before the appointment to arrange reimbursement.
- Call 877-685-2415. Say: “I have a ride to my medical appointment. How do I set up mileage reimbursement?”
- Confirm eligibility, driver requirements and the current rate. Obtain a trip number and the correct trip log.
- Bring the log to the appointment and have the healthcare office sign it. Complete all required driver, member and trip information.
- Submit it using the current form’s instructions; keep a copy. Modivcare’s published instructions require receipt within 30 days of the earliest trip listed. Confirm that deadline when arranging your trip.
Payment depends on eligibility, a properly arranged trip and complete documentation.
