What is Tailored Care Management?

Support that starts with you.

Tailored Care Management, or TCM, pairs eligible NC Medicaid members with a specially trained care manager. Together, you discuss your health, daily needs and goals, then coordinate the services and support that fit your life.

TCM is free and optional for eligible members. You can ask questions, choose whether to participate, and tell your care manager what matters most to you.

Who may qualify?

Ask your plan about eligibility.

TCM is available to eligible members through Tailored Plans or NC Medicaid Direct. It supports people with complex needs related to mental illness, substance use, intellectual or developmental disabilities, or traumatic brain injury. Some services may overlap with TCM, so your plan should confirm what you can receive.

Understanding 1915i services.

1915i, also written 1915(i), is an NC Medicaid program that helps eligible members get support in their homes and communities. Your Tailored Care Manager can help you understand the program and ask for an assessment.

What could it help with?

  • Learning or practicing everyday living skills
  • Finding and keeping a job
  • Moving from an approved setting into a private home
  • Providing a break for eligible caregivers through respite

The services available depend on your needs and the rules for each service.

How does my care manager help?

  1. Talk with you about your needs and complete the 1915(i) assessment.
  2. Help you find a provider and develop your care plan if eligible.
  3. Help coordinate any required approvals and the start of services.

An assessment and eligibility review come first. Each requested service has its own requirements. Read Trillium’s program explanation ↗
