Daily living and community skills
Community Living and Supports can help eligible people with intellectual or developmental disabilities or traumatic brain injury build skills for home and community life.
1915(i) is the name of an NC Medicaid benefit. It can provide home and community support to eligible children and adults with serious mental health needs, severe substance use disorders, intellectual or developmental disabilities, or traumatic brain injury. The goal is to help people live and participate in their communities.

Supported Employment or Individual Placement and Support may help a member find and keep a job. The service depends on the member’s needs and eligibility.
Community Transition may help an eligible member move from an approved setting into a private home. Individual and Transitional Support is another service for certain mental health and substance use needs.
Respite may give a caregiver a break while the member receives care. Age and other program rules vary by service.
These are examples. A member’s assessment, benefit plan, needs and service-specific rules determine which supports may be available.
The member generally needs NC Medicaid coverage through a Behavioral Health and I/DD Tailored Plan such as Trillium. Some people with NC Medicaid Direct or the Eastern Band of Cherokee Indians Tribal Option may also be eligible. Standard Plans do not cover 1915(i).
The member must meet requirements related to serious mental health needs, severe substance use disorder, intellectual or developmental disability, or traumatic brain injury, and need support at home or in the community.
An independent 1915(i) assessment reviews the member’s needs and helps determine eligibility and the services to include in a care plan or individual support plan.
People waiting for the Innovations or TBI waiver may be eligible. People already receiving those waivers are not eligible for 1915(i). Review the official eligibility guidance ↗
If you have a Tailored Care Manager or care coordinator, tell them which daily activities or community goals you need help with and ask about a 1915(i) assessment. You can also call Trillium Member and Recipient Services at 1-877-685-2415 and say, “I want to ask about a 1915(i) assessment.”
The assessment can be scheduled in person or through telemedicine. Trillium and your care manager can explain service-specific criteria, approval, and provider availability.
This page is general information. A call or assessment does not guarantee a particular service. For current requirements, use NC Medicaid’s 1915(i) guide ↗ and Trillium’s benefit information.
